– est indispensable pour s\u2019inscrire dans un club de badminton affili\u00e9 \u00e0 la FFBaD<\/span>
– permet de s\u2019informer des conditions d’assurance Generali inclues avec votre licence<\/span>
– permet de recevoir votre licence de badminton pour la saison demand\u00e9e<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t

Un certificat m\u00e9dical :<\/span>
– doit obligatoirement \u00eatre fourni pour une premi\u00e8re demande de licence adulte<\/span>

– est exig\u00e9 toutes les trois saisons cons\u00e9cutives pour les adultes<\/span>
– doit de nouveau \u00eatre fourni lors d’un renouvellement de licence en cas de r\u00e9ponse positive \u00e0 une ou plusieurs questions du questionnaire de sant\u00e9 dans l’intervalle des 3 saisons sportives<\/span>

Le questionnaire de sant\u00e9 :<\/span>
– doit \u00eatre renseign\u00e9 par le licenci\u00e9 \u00e0 chaque fois qu’un certificat m\u00e9dical n’est pas exig\u00e9 pour le renouvellement de la licence<\/span>
– doit \u00eatre compl\u00e9t\u00e9 avec s\u00e9rieux et sinc\u00e9rit\u00e9<\/span><\/p>

Le licenci\u00e9 doit attester avoir r\u00e9pondu par la n\u00e9gative \u00e0 chacune des questions du questionnaire de sant\u00e9.<\/span>
